Each seminar will accommodate 30 participants but if the number

| Letter of Invitation (for visa
application for admission into Korea) |

Participants requiring an official letter of
invitation in order to obtain a visa and authorization to attend
the 13th Congress of the WPACCM should contact the secretariat before
April 30, 2004. This letter is NOT a form of commitment on the part
of the 13th WPACCM organizer to provide financial assistance to
any participant. Expenses incurred are the sole responsibility of
participants.

| Registration Confirmation |

A written confirmation and official receipt of
your registration will be sent to you once your registration form
and payment are received.

Cancellation should be made in writing and sent
to the Secretariat of the 13th WPACCM. The following cancellation
policy will be applied:
